What The Pellet Insertion Process Is Like

Women may be considered for testosterone therapy in select situations, most commonly persistent low sexual desire causing distress after other contributors have been evaluated. Testosterone dosing in women should be conservative and carefully monitored.

Pellets may not be ideal for people starting hormone therapy for the first time, people who are very sensitive to hormone changes, people who need frequent dose adjustments, or people who have had side effects from hormone therapy in the past.

People with complex medical histories, fertility goals, cancer history, clotting history, uncontrolled cardiovascular risk, severe acne, androgenic hair loss, or unexplained symptoms need careful screening before pellet therapy.

Special Considerations For Women

Women need careful dosing with testosterone pellets because female testosterone levels are much lower than male levels. A dose that is appropriate for a man can be far too high for a woman.

Possible signs of excess testosterone in women include acne, oily skin, unwanted facial or body hair, scalp hair thinning, irritability, mood changes, sleep disruption, voice changes, or clitoral enlargement. Some androgen-related side effects may not fully reverse if exposure is excessive or prolonged.

For estradiol, many women do well with adjustable routes such as patches, gels, or creams. These allow the provider to raise or lower the dose more easily based on symptoms, side effects, bleeding pattern, and safety considerations.

If systemic estrogen is used and the uterus is present, progesterone or another appropriate progestogen is usually needed to protect the uterine lining. Progesterone may also be appropriate for some women who no longer have a uterus, especially when it supports sleep, mood, or overall hormone balance, but the reason for prescribing it is different than uterine protection and should be individualized.

Progestins are synthetic progesterone-like medications, while micronized progesterone is bioidentical to the progesterone the body naturally makes. These can have different effects and different risk profiles, so patients should know exactly which one is being prescribed.

FDA-Approved And Compounded Pellet Considerations

Hormone pellets may be commercially manufactured or compounded depending on the hormone and clinical situation. Compounded prescriptions are customized by a compounding pharmacy.

Estradiol, progesterone, and testosterone can exist as FDA-approved medications or as ingredients used in compounded prescriptions. The final compounded formulation is customized and is not reviewed by FDA in the same way as a commercially manufactured drug.

After insertion, follow-up may include symptom tracking, side effect review, lab monitoring at an appropriate interval, and review before any repeat pellet is placed. The next dose should not be automatic; it should be based on response and safety.

For women using estrogen therapy, uterine status and bleeding patterns should be reviewed. For men using testosterone, monitoring may include red blood cell count, prostate-related screening when appropriate, fertility considerations, sleep apnea symptoms, and cardiovascular risk factors.

Pellets may be a fit for patients who have a confirmed hormone need, tolerate hormone therapy well, prefer a long-acting option, and understand that dose changes are limited once the pellet is inserted.

They may be less ideal for people who are new to hormone therapy, need fine dose adjustments, have had side effects from hormones, are sensitive to medication changes, or want full control over stopping quickly.

Women should be cautious with testosterone pellets if they have acne, unwanted hair growth, scalp hair thinning, androgen sensitivity, PCOS with high androgens, voice concerns, mood sensitivity, or a history of excessive testosterone levels.

Men should be cautious with testosterone pellets if they have untreated severe sleep apnea, elevated red blood cell count, fertility goals, prostate concerns, uncontrolled cardiovascular risk, or unclear diagnosis of testosterone deficiency.

People with unexplained vaginal bleeding, active blood clots, certain hormone-sensitive cancers, severe liver disease, pregnancy, or high-risk medical histories may need alternatives or specialist coordination.

Risks, Side Effects, and Monitoring

Procedure-related side effects may include bruising, soreness, swelling, bleeding, infection, scarring, itching, tenderness, or pellet extrusion. Most insertion-site symptoms are mild, but persistent pain, drainage, redness, fever, or worsening swelling should be evaluated.

Hormone-related side effects depend on the hormone and dose. Monitoring should include symptoms, side effects, insertion-site healing, hormone levels when appropriate, CBC, CMP, lipids, blood pressure, glucose and insulin markers, breast or pelvic symptoms in women, prostate monitoring in men when appropriate, and reassessment before each repeat insertion.

Safety

Seek urgent medical care right away for chest pain, severe shortness of breath, fainting, one-sided weakness or numbness, sudden severe headache, vision changes, severe leg swelling or calf pain, coughing blood, severe abdominal pain, suicidal thoughts, or confusion. These symptoms should not be treated as routine hormone side effects.

Call your provider for non-urgent but important concerns such as insertion-site redness, drainage, fever, worsening swelling, persistent pain, acne, unwanted hair growth, scalp hair thinning, mood changes, sleep disruption, breast changes, abnormal bleeding, bleeding after menopause, or symptoms that feel too strong after pellet placement.

If you take medications for thyroid disease, diabetes, blood pressure, mood, seizures, fertility, blood thinning, heart disease, cholesterol, cancer treatment, pain, sleep, hormone therapy, or weight loss, pellet therapy should be coordinated with your prescribing clinician.
