DUTCH hormone testing is a specialty hormone test commonly used in functional medicine, integrative medicine, and hormone optimization practices. It is designed to show not only certain hormone levels, but also how some hormones are being metabolized and excreted.
The test typically uses small urine collections dried on filter paper. Samples are collected at set times, often across one day. This can provide information about hormone metabolites and, in some panels, the daily rhythm of cortisol and cortisone.
Hormones are not static. Estrogen, progesterone, testosterone, DHEA, and cortisol can vary by time of day, cycle phase, age, stress, sleep, medications, and hormone therapy. DUTCH testing tries to capture more of this pattern than a single snapshot.
That said, DUTCH testing is still one tool. It does not replace a full medical evaluation. A patient’s symptoms, cycle history, menopause stage, medication list, sleep, thyroid function, metabolic health, liver function, gut health, and safety risks all matter.
DUTCH testing is most helpful when the provider has a clear question, such as how estrogen is being metabolized, whether cortisol rhythm appears disrupted, whether testosterone metabolites are high, or how hormone therapy is being processed.
What Does DUTCH Testing Measure?
DUTCH testing may measure estrogen metabolites, progesterone metabolites, androgen metabolites, DHEA-related markers, cortisol, cortisone, and certain organic acids, depending on the panel ordered.
Estrogen markers may include estrone, estradiol, estriol, and estrogen metabolites. These can provide a view of estrogen breakdown pathways, which some providers use when evaluating estrogen symptoms, heavy periods, breast tenderness, PMS, perimenopause symptoms, or hormone therapy response.
Progesterone markers may help estimate progesterone activity, especially in cycling women when timed appropriately. Progesterone patterns can be relevant for PMS, irregular cycles, sleep disruption, anxiety before periods, and perimenopause symptoms.
Androgen markers may include testosterone-related metabolites and DHEA markers. These can be useful when evaluating acne, oily skin, unwanted hair growth, scalp hair thinning, low libido, low motivation, or testosterone therapy monitoring.
Cortisol and cortisone markers may help show a daily pattern of stress-hormone output. This can be relevant for people with morning fatigue, afternoon crashes, waking at night, feeling wired at bedtime, or poor stress tolerance. These results should not be used casually to diagnose “adrenal fatigue.” True adrenal insufficiency is a medical condition that requires proper medical testing.
How DUTCH Testing Is Different From Blood, Saliva, And Stool Testing
Blood testing is the standard starting point for many hormone and safety questions. It can measure circulating hormone levels, thyroid function, metabolic markers, liver and kidney function, blood counts, iron status, glucose, insulin, lipids, and inflammatory markers.
Saliva testing is sometimes used to evaluate free cortisol rhythm or free hormone patterns. Saliva can be helpful for certain questions, but it also has limitations and can be affected by collection technique, contamination, topical hormones, and timing.
DUTCH testing uses dried urine to evaluate hormone metabolites. This can provide a different kind of information than blood or saliva, especially about hormone breakdown pathways and cortisol metabolite patterns.
Stool testing is not a hormone test. It is used for selected gut questions, such as dysbiosis, inflammation, digestive function, pathogens, or microbiome patterns. It may be relevant when hormone symptoms overlap with gut symptoms, constipation, diarrhea, bloating, or poor nutrient absorption.
No test type is best for every situation. A provider may use blood testing for safety and baseline markers, DUTCH testing for hormone metabolism questions, saliva testing for selected cortisol questions, and stool testing when gut symptoms fit.
When DUTCH Testing May Be Worth Doing
DUTCH testing may be worth considering when symptoms suggest a complex hormone pattern and standard evaluation has not answered the main question.
For women, this may include perimenopause symptoms, PMS, heavy or irregular cycles, breast tenderness, migraines around the cycle, mood changes before periods, insomnia, hot flashes, night sweats, low libido, vaginal dryness, acne, hair thinning, or symptoms that fluctuate through the cycle.
For men, DUTCH testing is less commonly needed as a first-line test, but it may be considered in selected cases involving testosterone metabolism, DHEA patterns, cortisol rhythm concerns, or complex symptoms that are not explained by standard labs.
For patients using BHRT, DUTCH testing may sometimes be used to evaluate how hormones are being metabolized or whether dosing patterns appear excessive or insufficient. However, it should not replace symptom review, side effect monitoring, standard safety labs, or age-appropriate screening.
DUTCH testing may also be helpful when cortisol rhythm is a major question, such as waking tired, crashing in the afternoon, feeling wired at night, waking between 2 and 4 a.m., or feeling unable to recover from stress. The results should be interpreted carefully and in the context of sleep, blood sugar, stress load, medications, caffeine, and medical conditions.
When DUTCH Testing May Not Be Necessary
DUTCH testing is not necessary for every person with fatigue, weight gain, poor sleep, low libido, or menopause symptoms. Many people can start with a detailed history, symptom review, safety screening, and standard labs.
For example, a woman with classic hot flashes, night sweats, vaginal dryness, and menopause timing may not need DUTCH testing before discussing appropriate hormone therapy options. A man with possible low testosterone usually needs standard morning blood testosterone testing first.
DUTCH testing is also not the right first step for urgent or high-risk symptoms. Abnormal uterine bleeding, bleeding after menopause, pelvic pain, breast changes, severe depression, chest pain, neurologic symptoms, unexplained weight loss, or severe fatigue need appropriate medical evaluation.
It may not be useful when the result will not change treatment. Testing should not be done simply because it is available or because a large report feels more complete.
A responsible provider should be able to explain why the test is being ordered, what decisions it may influence, what its limitations are, and what other labs or screenings are still needed.
How It Works / What’s Involved
DUTCH testing is usually completed at home with a test kit. The kit includes collection cards, instructions, timing guidance, and mailing materials.
You collect small urine samples on filter paper at specific times. Depending on the panel, collections may happen upon waking, later in the morning, afternoon, evening, and bedtime. The samples dry on the collection cards before being mailed to the lab.
For cycling women, timing is important. Testing is often done during the luteal phase, after ovulation and before the period, when progesterone can be more meaningfully evaluated. For women with irregular cycles, menopause, hysterectomy, birth control use, or hormone therapy, timing instructions may differ.
Before testing, your provider should review medications, supplements, hormone therapy, topical hormone use, caffeine, alcohol, sleep, and cycle timing. Some products may need to be paused or timed differently only if your provider instructs you to do so.
After results return, the provider should review the report with you and connect it to your symptoms. The most important question is what is actionable: Does the result change hormone dosing, route, nutrient support, gut support, sleep strategy, stress plan, or safety monitoring?
Who It’s For And Who Should Be Cautious
DUTCH testing may be appropriate for people with hormone symptoms that are complex, cyclical, persistent, or not fully explained by standard labs. This may include PMS, perimenopause symptoms, menopause symptoms, low libido, acne, hair changes, irregular cycles, sleep disruption, cortisol rhythm concerns, or hormone therapy monitoring questions.
It may also be helpful for people who want a deeper look at estrogen metabolism or androgen metabolism when the results would guide a specific plan.
People who should be cautious include those who are pregnant, have unexplained vaginal bleeding, bleeding after menopause, active cancer treatment, complex endocrine disease, severe psychiatric symptoms, serious medical illness, or symptoms that require urgent conventional evaluation.
People using topical hormones need careful instructions because contamination can affect some hormone tests. Do not change or stop prescribed hormones without your clinician’s guidance.
DUTCH testing should not be used as a stand-alone reason to start high-dose hormones, aggressive detox protocols, or large supplement plans. It should be one part of a broader, safety-aware evaluation.
Risks, Side Effects, and Monitoring
The test itself is noninvasive and generally low risk because it uses dried urine samples collected at home. The main risks are misinterpretation, over-treatment, unnecessary supplements, or anxiety from a complex report.
Another risk is treating metabolites instead of treating the person. A marker may be high, low, or borderline, but the significance depends on symptoms, cycle timing, medications, hormone therapy route, medical history, and standard labs.
Some supplement or hormone changes based on DUTCH results can have side effects. For example, hormone dose changes may cause breast tenderness, spotting, mood changes, headaches, acne, sleep changes, libido changes, or changes in bleeding patterns.
Nutrient and “detox” support should also be used carefully. High-dose DIM, calcium D-glucarate, iodine, selenium, herbs, binders, or liver-support products may not be appropriate for everyone and can interact with medications or worsen symptoms in some people.
Monitoring should include symptoms, side effects, cycle pattern, bleeding pattern, sleep, mood, libido, skin changes, hair changes, energy, standard labs when appropriate, and age-appropriate screening. Repeat DUTCH testing is not always necessary unless it will change the plan.
Safety
Seek urgent medical care right away for chest pain, severe shortness of breath, fainting, one-sided weakness or numbness, sudden severe headache, vision changes, severe abdominal pain, suicidal thoughts, confusion, or symptoms that are rapidly worsening.
Call your provider for non-urgent but important concerns such as abnormal bleeding, bleeding after menopause, breast changes, severe mood changes, worsening anxiety or depression, new acne or hair loss after hormone changes, persistent insomnia, pelvic pain, medication side effects, or symptoms that worsen after starting supplements or hormone therapy.
Do not start, stop, or change BHRT, testosterone, thyroid medication, birth control, antidepressants, sleep medication, high-dose supplements, detox products, or binders based only on a DUTCH report. Safe care depends on your symptoms, medical history, medications, standard labs, screening status, and full clinical picture.
